Registration (Adults)
If you are at least 18 and looking to be cast as an extra in a movie or HBO series
you will need to print out one of my cards, fill out the information and
attach a 4X6 snapshot of yourself to it. If you are a minor or are
looking to cast your child you will need some additional paperwork.

your paperwork. Once all information is complete please

you are sending registration information can you please not overnight it
or fed ex it. This is just a mailing address, not an office.
If you don't have a
printer or cannot open either a PDF or JPEG Picture you will need to

Registration (Children)
Children under the age of 16 will need to fill out
and receive a work permit in addition to filling out the registration
form in order to participate as an extra. Illinois law requires
production companies to have all permits of children on file during
filming. You may acquire a work permit at a local high school or
school district office. You will need to print a letter of intent
before receiving your work permit to show proof of work. This is
the paperwork you will need to receive a permit:

You should be able to receive the work permit the
same day as application.
